Driver Authorization Form and Expectations
Use these forms to drive for a field trip or be a chaperone.
Livescan Fingerprinting Form
For all volunteers that need to be fingerprinted (ie. volunteers that
work directly with students and are NOT in the presence of a teacher/staff member at all times).
Medication Administration Form and Cover Letter
In order for your student to receive medication on campus, JAA must receive this form from the physician.
